Why would an older small breed dog become obsessive about hygiene?

I have no idea what behaviors you think are indicative a very human mental state, but guess it has something to do with licking/chewing?

Get thee to a vet with a description of specific behaviors like:

(3) There is a small lump near my dog’s anus and he chews on it several times a day to the point that the fur is gone.

(2) My dog is licking her vagina whenever she is sitting or laying down but I haven’t noticed any discharge.
